Root
Cause Analysis has now been widely adopted in the engineering field to assist
in finding the cause(s) that leading to the occurrence of an incident, but many
engineering practitioners find it difficult to determine the actual ROOT
cause.
In
this talk, Ir Dr Lim will share with the audience the important techniques
needed in a Root Cause Analysis, in particular, how to collect data following a
major incident and the process in identification of the problem(s), and how one
may apply the cause and effect principles to convert the data so collected into
knowledge that one may apply to understand the factor(s) leading to the
occurrence of an incident. Some case
studies will be presented to help explain the various steps involved in the
investigation process.
Speaker
Ir Dr Eric C.H. Lim graduated with a B.Sc. (Hon)
Degree in Mechanical Engineering form the University of Hong Kong. He obtained his PhD Degree from the same
department with research interest focusing on behaviour of materials. He then worked as a full time consultant
providing independent technical consulting services to the industry since 1996
and to date, he has completed more than 2500 investigations. Over the years, he has been involved in the
investigation of major engineering incidents in Hong Kong and Asia Pacific
Region. He also appeared as an expert
witness in the Court of Laws of Hong Kong, Beijing as well as Macau on more
than 300 cases.

charge and prior registration is required. The number of participants is
limited to 60. Applications will be accepted on a first-come-first-served basis
with priority given to members of MMNC Division. Only on-line registration via
